Delivering Personalised Care: Turn personalised care into everyday practice - confidently and with impact

Description

The Delivering Personalised Care program equips clinicians with practical skills, tools, and strategies to confidently embed personalised care and patient partnership into everyday practice.

Co-developed with clinicians and consumers, the program combines evidence-based learning with real-world application to help participants strengthen partnerships with consumers, improve patient outcomes, and align with national regulatory requirements and safety and quality standards.

Participants will engage in self-paced webinars, practical consultation tools, self-reflection exercises, and downloadable resources designed to build knowledge and confidence in delivering personalised care. The program also supports alignment with Patient-Reported Measures (PROMs, PREMs) and value-based care principles, contributing to accreditation readiness and measurable practice improvement.

Each participant receives a comprehensive downloadable featuring an 8-step personalised care consultation framework, practical tools, and curated links to key health reforms and Partnering with Consumers standards. These resources enable participant to fast-track implementation and sustain change in their own practice.   


Learning Outcomes

  1. Define the concept of personalised care and explain the two key elements that work together to deliver it
  2. Describe the purpose and key differences between Patient-Reported Outcome Measures (PROMs) and Patient-Reported Experience Measures (PREMs) and explain their role in supporting shared decision-making in clinical care
  3. Apply two practical strategies to incorporate PROMs and PREMs into routine consultations to better understand what matters most to patients and tailor care accordingly
  4. Apply informed shared decision-making techniques to co-design care plans with patients that align with their goals, values and lived experience
  5. Identify common barriers to personalised care at the clinician, patient, and practice levels and implement practical strategies to overcome them
  6. Assess personal consultation style using the 8-step personalised care consultation model and identify at least two areas for improvement in delivering care in partnership with consumers
  7. Integrate patient goals, values and outcomes that matter into care delivery using patient-reported measures and language that supports trust, inclusion, and patient activation
  8. Identify how adopting a patient partnership approach can improve outcomes, increase patient engagement and enhance clinician wellbeing and efficiency
  9. Identify gaps in your own consult by developing your own personalised care delivery plan to embed partnership-based practices you’re your consults using patient-reported measures.
