How to implement a total carbohydrate reduction diet for a patient with metabolic dysfunction

By completing this course, health professionals will be able to:

  • Understand the principles of implementing a therapeutic carbohydrate restriction (TCR) diet for patients with metabolic dysfunction, including the definitions and distinctions between low-carbohydrate, ketogenic and TCR diets.
  • Identify appropriate candidates and contraindications for TCR, and recognise when to exercise caution in clinical practice.
  • Conduct effective dietary assessments, including taking a quick diet history and identifying sources of hidden carbohydrates and sugar addiction patterns.
  • Educate patients on practical dietary strategies, such as prioritising protein, choosing above-ground vegetables, and using fats to promote satiety.
  • Guide patients in reading nutrition labels and tracking carbohydrate intake to support self-management and adherence.
  • Monitor and manage potential side effects (e.g., fatigue, cramps, palpitations, constipation, headache) and provide advice on electrolyte and hydration needs.
  • Safely deprescribe medications for diabetes and hypertension, with a focus on insulin, sulfonylureas, SGLT2 inhibitors, and diuretics, including protocols for medication reduction and monitoring.
  • Interpret changes in clinical markers, including glycaemic control, lipid profiles, liver function, and blood pressure, following a dietary intervention.
  • Support patients with ongoing follow-up, including regular review of blood results, monitoring for improvements in metabolic parameters, and adjusting care plans as needed.

These outcomes are designed to equip health professionals with the knowledge and practical skills to implement and support TCR interventions for patients with type 2 diabetes and related metabolic
