Join Silverchain’s panel of aged care experts on 5 August to learn more about the End of Life Pathway, available under the Australian Government’s Support at Home aged care program.  Using a range of case studies, our team will explain the End of Life Pathway, including eligibility requirements, services inclusions, and how the pathway complements existing generalist and specialist Palliative Care services. 

This free, national webinar is designed for doctors, practice nurses and community care teams.
