Diabetes is the fastest growing chronic disease in Australia and affects approximately 5% of the population, with Aboriginal and Torres Strait Islander Australians almost four times more likely than non-Indigenous Australians to have type 2 diabetes. It is believed that around 30% of Australians with diabetes are undiagnosed.

• 134,000 Australians are currently living with type 1 diabetes.
• Represents 10 per cent of all cases of diabetes and is increasing each year.
• Almost 1.3 million Australians are currently living with type 2 diabetes.
• Represents 85 to 90 per cent of all cases of diabetes and is increasing each year.
• Almost 60 per cent of all cases of type 2 diabetes can be delayed or prevented with changes to diet and lifestyle.

This new audit is unique in the education realm as it not only focuses on identifying prediabetes but also provides an alert system for clinicians. This system will assist clinicians and give them the tools to assist in the identification and recall of your patients with prediabetes and assists in the monitoring of patients already diagnosed with diabetes type 1 or 2. Only accessible via our online secure Medway portal audit reports e-referrals sent direct to the patient and all patients recall reports who are identified will be available 3 monthly this can then be printed and cross referenced in the surgery setting. The audit will occur each calendar available JAN- DEC.

NB: Gestational Diabetes will not be included in this audit.

Learning Outcomes

1. Recognise patients with impaired blood glucose levels that are higher than normal
2. Develop and implement a multipronged, multidisciplinary care plan tailored to each prediabetic patient
3. Review and identify processes related to patient follow-up and abnormal diagnostic results to ensure nothing is overlooked
4. Demonstrate and educate patients on appropriate glycaemic targets for individuals with type 1 and type 2 diabetes, considering factors such as age and comorbidities
5. Create systematic approaches to help patients with type 1 and type 2 diabetes achieve their glycaemic targets, individualizing their HbA1c targets
6. Clinicians can review their current practice standard, against best practice guidelines to achieve better support and care for patients.

Details

Following registration of the audit access will be given into our secure Medway site where clinicians will be sent an email alert when reports and patient follow up report is delivered, this will occur 3 monthly throughout the calendar year.

Added advantage with our online portal clinicians will be able to visualise graphically their patient's path through prediabetes and Diabetes Mellitus.

Performance review activities for this audit will include clinicians to complete an evaluation of findings at 6 and 12 months. Constant reviews with reporting 3 monthly and reviewing their patient follow up forms will be self-reflecting in regard to their patients' exhibiting signs of prediabetes and assess how well existing patients are managing their Diabetes Mellitus type 1 or 2.
